Patient-centred care is a buzz term, but what is it really and how do you know you’re getting it right? Diabetes WA and Steno Diabetes Center Copenhagen have partnered to bring you this interactive workshop to enable GPs and practice nurses to explore what patient-centred care looks like and importantly, what it doesn’t look like. Presented by Dr Nana Folmann Hempler and Dr Annemarie Varming from Steno Diabetes Center and the DESMOND Australian Team from Diabetes WA, this workshop will focus on self-reflection and evidence-based educational tools that support health professionals to engage in patient-centred conversations.
The Diabetes Management Research group at the STENO Diabetes Center focuses on developing and evaluating research based and innovative methods in patient education, social support for patients, and support related to work, life and chronic disease. The research group has developed several patient education concepts including The Balancing Person and The Health Education Juggler.
