How do you apply?
Diabetes Australia has changed the application process for Kellion Victory Medals. All Kellion Victory Medal applications must now be submitted to Diabetes Australia, rather than directly to Diabetes WA.
The new application method is via the online form submission portal (soon to be established) at www.diabetesaustralia.com.au/kellion.
Where needed (and in the interim), applicants and their supporters and health professionals can download a hard copy of the application form from the above webpage, complete and attach supporting information before submitting to Diabetes Australia via email to KellionVMS@diabetesaustralia.com.au or by post.
