Endocrinologist advice for your diabetes patients

One connection. Complete diabetes care.

Our Connect service supports GPs with diabetes management through phone calls and virtual case conferences with our endocrinologists

It is designed to:

  • respect the GP–patient relationship

  • support, not replace, GP clinical judgement

  • complement existing referral pathways

  • provide practical input when it may be helpful

Your patient remains under your care, and any treatment decisions remain yours.

What is Diabetes WA Connect?

GPs can call Diabetes WA Connect when:

• A patient’s glucose levels remain unstable despite treatment
• You are considering starting or adjusting diabetes medications
• A patient may need specialist referral but you would like advice first
• You need support managing diabetes in pregnancy
• A patient has complex comorbidities affecting diabetes care
• You want to explore multidisciplinary support options

Specialist diabetes advice when you need it

Diabetes WA Connect provides timely endocrinology advice to support the management of diabetes in primary care.

A short discussion with an endocrinologist can help clarify treatment decisions, support complex cases and connect your patient with the most appropriate services.

No referral required. No cost to patients or practices.

How does it work?

Phone advice

Speak directly with an endocrinologist to discuss a patient case.

Call 9436 6270 or book a 30-minute call-back window online.

Key points:

• The patient does not need to be present
• Advice is based on information provided during the call
• Follow-up calls can be arranged if needed

GPs can self-log RACGP/ACRRM Reviewing Performance CPD hours for case discussions, including preparation and reflection on advice.

Multidisciplinary case conference (MDCC)

For more complex cases, GPs can book a virtual MDCC.

Case conferences are attended by:

• a Diabetes WA endocrinologist
• a Diabetes WA educator
• an Aboriginal health practitioner (if requested)

The GP remains responsible for the patient’s clinical care.

MBS item 735 available

Case conferences may also be booked in extended blocks or used as group learning sessions for multiple GPs.

More than endocrinology advice

Diabetes WA Connect also links GPs and patients to the wider Diabetes WA support network.

This includes:

• credentialled diabetes educators
• dietitians
• telehealth services
• referrals to the Diabetes WA clinic
• the Diabetes WA helpline
• local community services and programs

This helps ensure patients can access the right support at the right time.

Targeted education events

The Diabetes WA Connect team can deliver diabetes education webinars tailored to the needs of your practice.

Practice team support

Credentialled diabetes educators can support practice teams to identify and address diabetes care needs within their patient population.

Aboriginal health practitioner support

Our Aboriginal health practitioners can assist practices and the Connect team with culturally safe care and community engagement.

Diabetes WA Connect information sessions

Practices can book a 30-minute introduction session to meet the Connect team and learn how the service can support their patients.

Our Team

Endocrinologist: Dr Gregory Ong MBBS (WA), PhD (MONASH), FRACP

Dr Greg Ong has clinical appointments at Fiona Stanley Hospital, Fremantle Hospital, and Sir Charles Gairdner Hospital where he previously served as clinical lead (Director) of Acute Internal Medicine. Greg can assist you with the management of any aspect of adult diabetes including diabetes in pregnancy.

Endocrinologist: A/Prof Seng Khee Gan, MBBS (WA), BMedSc (WA), PhD (UNSW), FRACP

Dr Seng Khee Gan has a clinical appointment at the Department of Endocrinology & Diabetes at Royal Perth Hospital, where he has been a previous Head of Department, and has a private practice.  He is also a Clinical Academic at UWA Medical School and has interests in inpatient and outpatient adult diabetes care, across the age range from young adults to the elderly.  He is active in collaborative diabetes research projects, as well as in medical education.

Multidisciplinary case conferences

GPs may claim MBS item 735 when coordinating a multidisciplinary case conference.

Continuing professional development

GPs can self-log RACGP/ACRRM CPD hours for:

  • case discussions with an endocrinologist
    participation in multidisciplinary case conferences
    preparation and reflection on clinical advice
