Team Care Arrangements
As part of a Team Care Arrangement (723) or review (732) you may refer your patient to one of our Credentialed Diabetes Educators or Accredited Practising Dietitians. Our Credentialled Diabetes Educators have many years experience and can provide your patient with tailored education, at both our Subiaco and Belmont offices, upon the following:
- Self blood glucose monitoring
- Oral hypoglycaemic agents
- Insulin initiation and titration
- Sick day rules
- Hypoglycaemia recognition and management
- Complication risk management
Our Accredited Practising Dietitians can, in addition, provide your patient with a nutritional assessment and advice tailored to their individual needs in addition to education upon the following:
- Influence of nutrition on blood glucose control
- Carbohydrate counting and information
- Weight management
- Blood lipid management
- Related health issues
- Complication management
- Hypoglycaemia recognition and management
All of our Credentialled Diabetes Educators and Accredited Practising Dietitians hold Medicare Provider Numbers. Please be aware that there is a gap, payable by the patient, involved in the delivery of our services as part of a Team Care Arrangement.
