We are delighted to share our Strategic Plan, which is designed to build a sustainable future for generations to come, so that every West Australian affected by diabetes can access the care and support they deserve.
Our strategy is anchored in five key pillars:
To engage and empower the community and elevate consumer voices.
To research and innovate to improve health outcomes and quality of life for people living with diabetes, to promote a more sustainable healthcare system, and to support advancements in diabetes care by building strategic partnerships that drive research, innovation and real-world impact.
To strengthen the health workforce, building capacity and confidence to better support people and prevent complications in an increasingly complex healthcare environment.
To expand access and support to provide consumers with clinical services and health programs to help them live well with diabetes.
To advocate and collaborate for systemic change to tackle the unmet and emerging needs of the diabetes community and make diabetes support more equitable, accessible and sustainable across Western Australia.
Diabetes WA CEO Melanie Gates says: “We see the difference that community-based support can make every single day. The work done by community health organisations like ours often makes the difference that keeps someone well at home and out of hospital.
Too often, conversations about fixing the health system start at the hospital door. To build a system that is truly sustainable and better able to meet future demand, we need to start much earlier. We need to keep people healthy.”
