Become a SMARTs facilitator

Empower your community through diabetes self-management

Evidence has shown that diabetes self-management education improves health outcomes, lowering complications and reducing hospital stays.

To support the delivery of these critical diabetes services across Western Australia we will be partnering with local health professionals to deliver structured self-management education in their communities.

The SMARTs programs

The SMARTs programs have been delivered in WA over the past 15 years and have recently had their content and activities refreshed. The aim of these practical programs is to build the capability of people with diabetes to manage their condition with confidence.

  • CarbSMART helps participants understand carbohydrates, make informed food choices and manage their blood glucose with confidence.
  • FootSMART helps participants builds skills and routines to care for their feet and reduce the risk of diabetes-related complications.
  • MedSMART helps participants understand their diabetes medications, use them safely and feel confident managing their treatment.
  • ShopSMART teaches participants how to read food labels and make informed supermarket choices to support their health.

The importance of the facilitator role

 Our non-judgemental person-centred philosophy of care aims to empower individuals to manage their diabetes complications. At Diabetes WA, we believe a statewide network of skilled health professionals enables more Western Australians living with diabetes to self-manage their condition close to home supported by clinicians they already know and trust. 

What you'll do

Attend training or upskilling. Facilitate two or three-hour self-management workshops for people living with diabetes and their support people in your local community.

What we'll do

Schedule programs, recruit participants, provide attendance lists, send reminders, provide customer support for attendees, provide program delivery kits and workbooks.

Who we need

Registered health professionals in all corners of the state with capacity to deliver regular group workshops as a sub-contractor or as part of their scope of practice (in consultation with their management).

Pathways

 If you have been a SMARTS delivery partner with Diabetes WA before, we are keen to discuss renewing our agreement and offer you refresher upskilling of the new content. If you are new to SMARTS facilitation, get in touch via the form below and we can discuss program delivery and training.

Frequently Asked Questions

Registered health professionals with capacity to deliver regular group workshops as a sub-contractor or as part of their scope of practice (in consultation with their management).

For existing or past facilitators the upskilling course is four hours.

Two days

Time to deliver – sessions run for two or three hours. CarbSmart: 3 hours, FootSmart & MedSmart: 2 hours, ShopSmart: 2.5 hours. 

You can deliver programs according to your availability and the need arising in your community. There is a minimum requirement of three programs each year, but we can discuss an arrangement that is mutually suitable. 

Training and mentoring are free. This project is supported by the Western Australian Department of Health.

Training will take place at Diabetes WA or online. 

That’s fine. We are happy for facilitators to stick with their areas of expertise and comfort.
